Before requesting a background check you certify that:

• The candidate has elected to share his/her report with you; or
• A clear and conspicuous disclosure will be made in writing to the applicant who is the subject of the report to be procured in a document that consists solely of the disclosure, that a Consumer Report may be produced for the employment decision; and,
• the candidate has authorized in writing the procurement of the Consumer Report you are requesting.
• will provide a copy of the report and "Summary of Rights" to the applicant/employee before taking adverse action; and,
• The information is to be used in compliance with the Fair Credit Reporting Act (“F.C.R.A.”) and all other applicable laws. Particularly, you are requesting this information under the guidelines of Section 604 and all of its requirements
• You will not use the report in violation of any equal opportunity laws or regulations. See 15 U.S.C. 1681b.
